I’ve attached the form we use which also state our policy for Incompletes. Prof’s have to verify that they have received enough information to justify that the incomplete meets the policy requirements.
Just because there is sometimes confusion around the statement, The “grade will change to” part of the checklist is in reference to courses that have non-standard F grading such as courses that assign No Pass (NP) etc. instead of an F.
